IMPORTANT : SHA has reserved a limited number of rooms at a very special rate at the host hotel, the Hilton Québec, situated right next to the conference venue, the Québec Convention Centre. Both are at the entrance to historic Old Québec. You will be able to book your hotel room through the online conference registration web page. The room rate is CAD $129 per night (plus tax) for single or double occupancy. This rate will be available from January 2 to January 14, 2014.

The full refund of your registration fee less CAD $50 will be made for cancellations received before December 1^st, 2013 only. No refund will be made for cancellations made after December 1^st 2013.

• Write your name, as you would like it to appear on your conference badge.
• Registrations will not be accepted by telephone.
• Registrations for the conference cannot be confirmed if the registration is not accompanied by the appropriate payment.
• Registrations and/or payments received after the early registration deadline of December 1, 2013 will be viewed as regular registrations and the full regular registration fee will be charged.
• Online registrations received after December 24, 2013 will not be accepted, and participants will have to register on-site.
• Hotel reservations at the Hilton cannot be made if you are not registered to the Conference.
• The local organizers or organizing secretariat will not accept liability for any kind of money loss.

Once your online registration has been successfully processed, you will receive confirmation by e-mail within 3 working days. If you did not provide your email address, your confirmation will be faxed or mailed to you. In order to avoid any problems when registering at the Conference, participants are requested to carry with them the complete documentation of payment of the registration fee.

The on-site registration fee will be charged after December 1, 2013. Participants registering on-site cannot be guaranteed a delegate’s bag with the final program and other conference materials. Availability will depend on the number of on-site registrants.

You will receive a name badge with your registration documentation. This badge has to be worn at all times to enable you to access all sessions and exhibition area.

Currency exchange charges or bank collection fees are not included in the total costs and are to be paid by the registrant. Currency exchange charges or bank collection fees are the whole responsibility of the registrant. Registration and accommodation are only complete if the form is submitted together with registration payment, and if the bank draft for the correct amount or the credit card information is approved.

If a collective payment has been made, please indicate the name of the remitter and attach a list of participants in order to accurately confirm their individual payments.

Substitutions are welcomed if you are unable to attend the Conference and would like to send another person in your place. There will be a CAD $50 charge. Please advise us by email conference@conferium.com with the name and personal data of your substitute. Substitutions will also be accepted on-site. Please note that only one person may attend per registration. You may not split a SHA2014 registration (i.e. the same person must attend the entire program on a single registration.)
